REFEREE INVOICE TEMPLATE
Can all referees please use this template to seek payment from the Zone organized rep games. If you are refereeing for the Rebel Sport Junior Premier League - please indicate this clearly and boldly on the invoice.
Right click the link below and "Save target as" and save it on your computer. You can then fill in your details for each game and send it in to referees@sunshinecoastfootball.com.au
This will ensure that payment is quick and easy and assists us in keeping track of who is refereeing which games.
BLUE CARD
Every referee who will turn 18 during the season needs a Blue Card from Qld Goverment. Fill the B Part ONLY and then come to the office with 100 points ID and all the forms. We will send the application to the Child Services and after a few weeks you will receive a Blue Card in the mail.
The Blue Card is valid for 2 years so check your existing Blue Card for the expiry date.
The Blue Card is free for volunteers (referees).

You need to fill this form to be able to get appointed to games. Download the form and bring it with you when you come to register as a referee OR you can also fill one at the office, I have copies available.
Remember to tell us if you are playing for any team or coaching / doing something else for that team, so we know where you are during the season and you will get your games on the same venue where your team is performing each weekend.
Give us all the details of the team, just writing Kawana or Maroochydore doesnt help us at all. Club+Team+Division+Age Group+"Special name"=Buderim U12/1 Black.

- Red Card for a player
- Team Official (coach, manager etc) was sent off the bench
- Something else happened that need to be reported
